The general manager of the Colorado State Fair, Sarah Cummings said, "They bring their own carnival inspectors to inspect those and then the Colorado State Fair goes in on an additional level and brings an inspector contacted through us". Ohio Department of Agriculture records provided to The Associated Press showed passing marks on inspections of about three dozen items, including possible cracks, brakes, proper assembly and installation. The law firm said Friday it has retained investigators to reconstruct the malfunction and determine how and why it happened. It said Fire Ball had been checked by its staff and independent inspectors before the fair opened. It can hoist riders up to 40 feet (12 meters) in the air and spins them at 13 revolutions per minute, according to Amusements of America.

The ride involved in the fatality was made in 1998 by KMG, a Danish company. "I knew the rides were risky, but I rode them anyway". None has had a serious malfunction before, the company told the AP.

The Denver Channel reported on Thursday that the Arapahoe County Fair pulled another ride, The Freak Out, in an "abundance of caution".

The U.S. Consumer Product Safety Commission is also investigating. It estimates there were 30,900 injuries associated with amusement attractions that required emergency room visits previous year.

It's one week week before the opening of the Indiana State Fair fair starts and crews are busy setting up.
